Str8 Teeth via their website is providing information about a product and service only. That service is the provision of clear aligners through a dentist. Str8 Teeth itself will not provide you with a product or service. Str8 Teeth will take your contact details and information as you provide them on this website and supply them to a dentist who will then contact you to make arrangements for an initial consultation and treatment plan for clear aligners where appropriate.
Health Insurance Terms and Conditions
When Using Private Health Insurance Str8 Teeth and any dentist affiliated with Str8 Teeth that may provide you with services, has no control over what your private health insurance pays towards your dental treatment.
The dentist will be able to process your health fund card on their HICAPS machine as a complimentary service. When they do this, HICAPS will pay any benefits to the dentist directly on your behalf to save you having to claim back the benefits yourself after paying the dentist in full. However, what your health fund pays towards your dental treatment is entirely between you and your health fund. It is your responsibility to contact your health fund regarding their terms and conditions, especially the benefits they provide and limits and restrictions on these. Str8 Teeth, and any dentist affiliated with them, does not accept responsibility when your health fund does not pay the benefits that you think they should. You are responsible for paying your account whether your health fund contributes to it or not.
Initial Dental Appointment Terms and Conditions
For $199 you will receive a comprehensive dental examination, small x-rays to check between teeth, a referral to an x-ray clinic for big x-rays to check the jaws, photos of the face and teeth, and impressions of the teeth to get a clear aligner treatment plan from one of our dentists. If you have health insurance with valid dental cover, we can accept your health insurance rebate as full payment for this, with you paying $0 out of pocket. You will need to bring in your health insurance card for us to process on our HICAPS machine at our dental practice at your appointment. If your health insurance does not cover these when your health insurance card is processed by us, you will have to pay $199 yourself instead.
To receive this offer, you need to mention it at the time of booking your appointment. Services will only be provided if suitable and appropriate. If you are not suitable for clear aligner treatment in the short term, you will be given a treatment plan and quote for additional treatment where appropriate to get you ready for clear aligner treatment. If you are not suitable for clear aligner treatment at all, you will be given a treatment plan and quote for alternative treatment options such as braces. In these cases, referral to an x-ray clinic for big x-rays of the jaws, photos of the face and teeth, and impressions of the teeth may be delayed until after additional treatment is carried out or not provided at all if clear aligner treatment cannot be done.
Clear Aligner Packages Terms and Conditions
The clear aligner package most suitable and appropriate for your case will be provided to you after examination by the dentist and analysis of records of your mouth. The images provided are examples of mild, moderate and severe crowding and spacing only and used purely to help you understand their differences. Your dentist will discuss with you in detail which package is most suitable and appropriate for your case, the costs involved and what it covers.
